color=””]在[/fusion_dropcap]美國根據國家健康統計院的資料顯示，美國大約有1%的人口患有拇趾外翻，而且隨著年紀的增長，發生率會越高。而在臺灣65歲以上的女性，35%有拇趾外翻的病症，這種擾人的足部疾病女性為男性的10倍，尤其是穿窄鞋頭的高跟鞋女性，拇趾外翻的情況會更嚴重。通常嚴重的拇趾外翻都是長時間造成的，所以老年人的發生率比年輕人高，發生的原因可分為內部與外部兩種，內部因素包含了家族性的遺傳因子、扁平足患者以及腳趾頭的外型；外部因素為穿鞋不當及腳趾頭不當使用，而穿高跟鞋一直都被認為是導致拇趾外翻的最大原因之一。所以如果有拇趾外翻的情況，穿高跟鞋絕對會讓病症惡化。 . 《案例篇》鄭小姐是一位show girl，假日穿梭於各個展場，三C產品展、車展、家具展等等，要從事這一份工作還真不容易，除了要站一整天之外，鄭小姐最困擾的一個問題就是—穿高跟鞋。因為她的腳趾頭的大拇趾已經外翻，每次都說要去骨科檢查，可是一個接著一個的展場工作捨不得推辭的情況下拖延了就醫，這次鄭小姐因為拇趾外翻痛到無法穿鞋，終於願意來找眼鏡小醫了。鄭小姐第一句話就問：「拇趾外翻要開刀嗎？」眼鏡小醫不會直接回答「要」或是「不要」，而是告訴患者開刀之前先考慮三件事情—美觀、疼痛、穿鞋習慣，如果為了美觀、又怕疼痛，又像鄭小姐的工作都必須穿高跟鞋一樣的話，開刀是解決的方法。相反的，如果不疼痛、平常都穿拖鞋、也不覺得外觀不好看，眼鏡小醫則不建議開刀。鄭小姐去掛眼鏡小醫的門診是因為腳趾頭痛到無法忍受，而且也無法去工作，因為常常穿著窄鞋頭高跟鞋的她，腳趾頭在狹窄的鞋頭空間相互摩擦，現在已經痛到無法穿鞋了。眼鏡小醫說：「鄭小姐的情況叫做磨擦痛，其實不穿鞋就不會痛了，但是鄭小姐的工作是必須穿鞋子的。」另外一種拇指外翻造成的疼痛叫做拉張痛，這是因為大拇趾已經變形了，沒有正常的使用它，而使得變形的部位一直往外擴張產生疼痛感，這種情況即使穿拖鞋也一樣會痛。眼鏡小醫建議鄭小姐先考慮美觀、疼痛、穿鞋習慣這三件事情之後，再決定是否開刀，若不開刀的話，可以從穿戴輔具、做復健運動讓疼痛減緩。 . 《保健篇》大拇趾的關節側面向外突出，並且造成關節腔的黏液囊組織發炎，從外觀上看到的，就是大拇趾的側面有突出的囊腫，拇趾的趾頭往內傾斜的狀態，這就是「拇趾外翻」。既然大部份原因是因為穿窄鞋頭高跟鞋造成的，那就換鞋吧！例如穿完全沒有包覆性的夾腳拖可以嗎？眼鏡小醫表示，以夾腳拖的設計來看，它只能減少拇趾外翻的疼痛感，但是無助於肌肉力量的訓練，所以不建議拇趾外翻的患者穿夾腳拖來改善。正確的改善方式是：１．從小就要訓練正確使用腳趾頭。２．不要穿窄鞋頭的鞋子。所以選擇正確的鞋子穿也是一門重要的保健課題，下次就請眼鏡小醫教大家如何選擇鞋子，保護腳部的健康。
